  • To be used a Lasting Power of Attorney (LPA) must be registered with the Office of the Public Guardian.
  • Form LPA001: Notice of Intention to Apply for Registration of a Lasting Power of Attorney and Form LPA002: Application to Register a Lasting Power of Attorney are the required documents for registering an LPA.
  • LPA001 is the form used to notify the individuals named in the LPA of your intention to register the LPA.
  • Form LPA002 is the form used to apply for registration and must be sent to the Office of the Public Guardian.
